You can apply in three different ways. They include through the phone, mail or online. When applying online, you need to visit the National Plan & Provider Enumeration System official website.
While in the website, click on the link that says Apply Online for an NPI. This will lead you to a form where you need to fill in all the required information. Some of the data that is required includes the name and address of your establishment, your social security digits, provider type, contact person, licensing information and taxonomy code. When you are done, click on the submit button.
After the form is submitted, some digits will be auto generated. It is the tracking figure for your National Provider Identifier application. Write it down.
When that is done, wait for the National Provider Identifier figure. This can take a while, like 15 working days. If you do not receive it within that time span, place a call to the enumerator.
If you decide to use the telephone or mail, you will have to download and print the CMS-10114 form from the internet. Once you have the hard copy, fill in all the details that are required in the form. Mail it once you complete filling the information.
